CUPE Supplemental Fund -Application Form
CUPE 917 and 951 have set up a joint committee to distribute the CUPE Supplemental Fund as set out in the Letter of Agreements (CUPE 917 / LOU #12 and CUPE 951 / LOA #13).
Locals 917 and 951 have mutually agreed to use this fund to supplement the (Plan C) Orthodontics or Hearing Aids Health Benefit supplied by Pacific BlueCross (claimed once every 5 years) to a maximum of two-hundred, fifty dollars ($250) per member allotted on a first come, first served basis annually. It is not meant to replace the Health Benefit, rather to supplement it. The Joint committee comprised of two (2) CUPE 917 and two (2) CUPE 951 representatives appointed by the Union will administer the fund.

Termination Account
If you accumulate sick benefit credits in sufficient quantity, you may be eligible to transfer a portion
of them each year into a termination account and, upon terminating employment, except for cause,
be paid those credits in cash.
Article 25 Section E Section E – Termination Pay
Any employee who has accumulated at least twenty-six (26) days of sick benefit provision
may opt, in each subsequent year in which that employee’s account is equal to at least
twenty-six (26) days, to place any number of days of unused annual sick benefit provision
in excess of ten (10) days in a special termination account.
Any employee who has accumulated at least forty-four (44) days of sick benefit provision
may opt, in each subsequent year in which that employee’s account is equal to at least
forty-four (44) days, to place any number of days of unused annual sick benefit provision
in excess of five (5) days in such termination account.
Any employee who has accumulated at least one hundred and thirty (130) days of sick
benefit provision may opt, in each subsequent year in which that employee’s account is
equal to at least one hundred and thirty (130) days, to place any days of unused annual
sick benefit provision in such termination account.
Upon termination of employment except for cause, the days in an employee’s special
termination account to a maximum of two hundred and sixty (260) days shall be paid to
the employee at the rate of the average per diem rate of pay for the employee’s five (5)
highest paid years of service. Upon transfer to a position outside the CUPE Locals and
successful completion of probation in the new position, the special termination account
shall be paid out in full. Where the termination results from the death of an employee, the
account shall be paid to her/his estate.
Waiting Period for LTD Benefits
If you have sick leave entitlements remaining after the waiting period, the waiting period will be
extended to the date these benefits end, but not longer than one year after the disability starts.
You also have the option of using remaining vacation or Termination Account days to extend the
waiting period to the limit noted above. This allows you to maintain your full regular salary for a
longer period of time.
Current Year Termination Account Payout Request
I request to be paid out for my Current Year Entitlement (‘Earned’) for Termination Account as
indicated on my Employee Dashboard Leave Balance Information page. I understand this payout will
be added to my April 30th pay. Below is a downloadable form:
Sick Benefit Bank
The Sick Benefit Bank is a resource available to all members of CUPE 917 who are in need of sick time in excess of their annual allotment due to an extended illness, injury or disability. Every member contributes to the fund — each year you will note a small amount of your personal allotment is contributed to the fund — and the Sick Benefit Bank committee uses this fund when members apply using the form below.
Please be advised that Members are not required to use up their vacation entitlements before applying to the Sick Benefit bank.

Retirement
When you’re starting to think about retirement, there are a few steps need to take beforehand. Below are the steps they need to take to help make the retirement process a little easier on everyone.
- If you are planning on retirement in the next 5-years, please complete the attached Estimate Application, and forward the completed application to Pension Services in MWB, or by email at pensions@uvic.ca . Pensions asks that these requests be submitted at least 3-months prior to your retirement date, but no later than 1-month prior to your retirement date. The more lead time you can provide pensions, the more smoothly this process will go.
- At least 1-month prior to your retirement date, you must also inform the department of your impending retirement. I have attached a template letter that staff can use for this. This letter must be handed in to the Supervisor, with a cc to the Manager, who will then pass on the letter to me for the departments records. Once I have this letter, I can then complete the forms that pensions need to start formally processing the retirement. Again, the more lead time the staff member provides, the smoother the process.
- Members are also encouraged to contact the Benefits department for information on continuation after retirement. Staff can send these requests to benefits@uvic.ca .

What to expect when phoning for EFAP services
- Select one of the following options:
- Option 1 for individual urgent or crisis assistance
- Option 2 for standard appointment booking or re-booking
- Option 3 for Critical Incident Response (Groups)
- You will be asked a series of questions for the purposes of matching your situation to the right service and security/privacy assurance (such as your mother’s maiden name or date of birth)
- For safety reasons, every caller will be asked a question assessing the possible risk of harm to themselves or others at the beginning of the call
What is the cost?
- This is a 100 per cent employer covered benefit for eligible employees and their dependents.
- The EFAP benefit covers the cost of short-term counseling sessions. If longer term or more specialized counseling is required, you will be referred to a community resource that you can afford.
